Intelerad and FINAO continue to work through final configurations to transition from Intelerad’s temporary server environment to the permanent environment, while minimizing disruption during the transition.

Intelerad Go live – Monday (7/17) @ 12:00 Noon

FINAO just received confirmation that the Monday go live for the permanent servers can occur at 12:00 Noon, to better accommodate the Monday morning surgery schedule.  Unfortunately, Intelerad was unable to perform this go live in the requested ‘off peak’ hours. 

FINAO Support contacted sites with the ‘single use’ password required to access the permanent environment. Individuals with active user names can then reset their permanent password. FINAO Support is available at 256.705.3462, if needed … or you can call me directly.

No changes to the current system will occur until Monday 7/17 at 12 noon. 

ROH Radiologists will be on-site at facilities for this go live and  access to the current temporary system will remain active during the go live and for a residual time thereafter.  Access to the current temp system during go live can occur as follows: (below)

1. Currently the url (https://pacs.finaosolutions.com) is pointed at the public IP address (142.215.19.50) for the temporary system.

2. At the time of go-live, the url  (https://pacs.finaosolutions.com) will be pointed at the NEW system public IP address.

3. IF someone still needs to login to the temp system, they would use the public ip address (https://142.215.19.50). The certificate will say “Not Secure” or Private but that is only because of using the public IP instead of the named URL. Below are screenshots of using the temporary system public IP address:

Go live – DELAYED

The planned ‘go live’ for the permanent Intelerad system, which was initially scheduled for tomorrow (7/13/2023), is delayed. 

Additional work is needed to ensure all Intelerad site and provider configurations are correct in final testing. 

The future ‘go live’ date will be circulated via email and this information will be posted to the Radiology of Huntsville website when the date is confirmed. 

The temporary system will remain active until ‘go live’ with permanent servers. Thank you for your continued patience as we recover.

Imaging volumes increased significantly following the Memorial holiday. These increasing volumes and the scores of comparisons retrieved for needed interpretations impacted the speed of the temporary system materially earlier this week. FINAO is aware of these events and the team was actively working with Intelerad during these events. The permanent server environment is significantly more robust. Currently, ROH operates on a single, large temporary server. This will be replaced and then mirrored with twelve (12) corresponding servers. This additional horsepower is needed and Intelerad is actively loading software and configuring this environment. ROH is continually assured that this project is top priority for Intelerad, and we anxiously await the ETA.

The pending Intelerad worklist is largely within expected parameters, with the vast majority of the studies completed on or after May 25th. Radiologists continue to work through the remaining studies that may be awaiting prior imaging / reports.

ROH sincerely appreciates the substantial efforts technologists provided to help get the worklist to this point. Your hours of work retrieving priors, distributing reports, and performing all needed tasks to help care for imaging patients have not gone unnoticed. On behalf of ROH and its most grateful doctors, please accept our sincere thanks. We could not have recovered to this point without you and your dedicated efforts.

Nonetheless, we know much work remains to restore full PACS services. These efforts continue and will continue through the holiday weekend.

Lastly, please join us in pausing this Memorial Day weekend in remembrance of the brave soldiers and servicemembers who gave the ultimate sacrifice for the country we are fortunate to call home. ROH radiologists convened via a one hour Zoom meeting to strategize the best pathway to address all studies requiring a final report. ROH learned that the manual connection to the study archive is stable and FINAO is working toward seamless interface. Connecting access to the archive with prior studies is paramount. The process to manually request an archived study is, as follow:

The workflow for these manual, individual study retrievals is as follows:

1. Best Case: Physician requests needed imaging from the archive with patient Accession Number (available on previous reports)

2. If no patient Accession Number is available: Forward full patient demographics, date of birth, MRN, date of study, location of study, etc. – FINAO staff will attempt retrieval without Accession Number

– If the patient has previous interpretations by an ROH radiologist, please copy ROH at bmccown@radhunt.com with  full patient demographics, date of birth, MRN, date of study, location of study, etc. and ROH should be able to retrieve the Accession Number.

HL7 connections and integration of the long-term storage archive into the new data centers are now the primary areas of focus.  Numerous HL7 connections were completed last night with more in progress.

UPDATE: FINAO Intelerad PACS

(May 5, 2023 – AM Update: for immediate release)

A planned Intelerad restart will occur today at 12:00 noon (Central). This restart will allow Intelerad to apply a ‘patch’ to the software and to update needed rule changes. Intelerad expects continued improvement in speed, stability, and performance following this update. This downtime should not exceed ~15 minutes. Any additional information regarding this restart will be relayed, as it is received.

All ROH radiologists are dictating and issuing reports, using the new Power Scribe 360 voice recognition platform. These dictated reports are available from the imaging facility. FINAO re-established an additional Mirth server late last night to address additional transmission of HL7 messages.

Radiologists will be working through the weekend to interpret studies from the worklist. Work continues to complete all necessary network connections and initial steps began to reestablish pathways to long term archived studies. These combined efforts will also continue through the weekend and ROH will post a midday update this Saturday (5/5) and Sunday (5/6) at:

PowerScribe 360 is active on a significant majority of ROH reading stations and is being used to dictate reports.  The reading stations remaining are being actively configured.  Dictation greatly improves radiologists’ ability to complete patient reports.

ROH radiologists have added additional reading hours in order to address the patient worklist.

FINAO will install a software patch at 12:00PM tomorrow (Friday) with stability and performance improvements to the Intelerad system.  A brief restart of the system will be conducted in relation to the install resulting in downtime of less than 15 minutes.

(May 4, 2023 – Morning Update – for immediate release)

FINAO successfully brought six (6) Power Scribe 360 servers along with a Mirth server on-line yesterday and radiologists began dictating reports using this voice recognition on Intelerad. Work to configure PS 360 for all radiologists continues. This voice recognition functionality tremendously improves radiologists’ ability to complete patient reports.

  • Networking efforts to reestablish all needed connections continue daily.

Primary efforts today are focused on achieving 100% voice recognition by radiologists for patient reports and adding functionality modules in Intelerad to assist with patient workflow.

Establishing Power Scribe 360 functionality was critical to this recovery project. As all radiologists can launch their PS 360 platform throughout today, ROH expects to see quick progress in addressing the patient worklist.

Lastly, ROH, FINAO and the entire team are acutely aware of the need to establish connection to the long-term archive for study retrieval. Initial preparations for these connections are occurring and this will quickly become a primary focus.

UPDATE: FINAO Intelerad PACS

(May 3, 2023 – Morning Update – for immediate release)

Final configurations occurred into the night for the new Power Scribe 360 voice recognition platform. Archived report templates are being loaded and we expect a ‘go-live’ ETA for this system, imminently. Numerous imaging modalities and imaging center modality servers were fully connected to the FINAO network in the last twenty-four (24) hours. These sites now have full PACS Viewing with Preliminary Report capability, and these sites are working diligently to distribute Reports to referring physicians.

  • IT networking teams continue to establish connectivity across all imaging modalities and site modality servers to allow full PACS Viewing and interpretation.
  • Final steps preparing the PS 360 platform are underway with an ETA expected.

These are the next critical steps in working to establish the reading environment. Continued efforts to integrate connections to the long-term archive for needed comparisons will soon follow. Also scores of HL7 messaging connections to reestablish efficient report distribution must be addressed.

The Power Scribe 360 platform and its functionality significantly improves ROH radiologist’s ability to efficiently provide needed patient interpretations. We hope to forward a thankful announcement of this ‘go-live’ very soon.

May 2, 2023 PM Update

PowerScribe is undergoing final configuration and is now in the testing phase.  Templates are being migrated into the system.  Please be assured, a separate communication will be sent to all ROH radiologists as soon as PowerScribe can be used for dictation.

ROH radiologists began seeing increased functionality of the new PACS viewing system as additional sites are connected and preliminary interpretations are entered into the new system. Priorities for the IT teams are as follows:

  • Full image viewing and preliminary interpretation capability across all sites, including establishing all needed site connectivity.
  • Completing install and configuration of the new PS 360 voice recognition platform.
  • Reestablishing the full service PACS reading environment.

As these critical tasks are completed, the teams will address all HL7 message interfacing across all facilities for Orders and Reports. Unfortunately, providing definitive ETAs for completion is nearly impossible, as tasks are addressed continuously. As tasks are completed, our team members will communicate directly with facilities.

We want to assure our patients, referring, physicians, staff and leadership at imaging facilities that ROH increased its radiologist coverage both across sites and remotely to ensure timely interpretation of all urgent imaging. Unfortunately, routine imaging studies are interpreted as quickly as possible.

ROH radiologists remain available 24/7 for consultation and interpretative services. We anticipate material progress toward full restoration of the PACS system in the coming days. We regret that an exact timeline is impossible to predict at this time, as work continues minute by minute.

ROH is fortunate to have the support of this wonderful community and its clinicians and please know that excellent patient care remains top priority.

The configuration of PowerScribe dictation software made significant progress today with efforts ongoing through the night.  Additional information is expected in the morning on this installation.

Once PowerScribe is active and the new Intelerad network is connected to FINAO’s offsite long-term storage, final reads will be provided by ROH radiologists.

ROH Radiologists continue to provide preliminary interpretations, around the clock, via the new Intelerad connection. Work continues, on the follow:

  • Reestablishing connection to all remaining facilities and imaging centers to, initially, allow access for preliminary reports.
  • Full configuration of a new Power Scribe 360 server
  • Final configurations of the new Intelerad servers

Each of these tasks are addressed with the goal of achieving final interpretations for your patients and your facility as quickly as possible. As any of these tasks are completed, FINAO and a designated radiologist will directly communicate with your center.

Although monumental efforts occurred to reach this point, we understand your frustration and need for final interpretations for your patients. Please know that our IT teams worked tirelessly through the weekend, and they will continue their efforts until full service is restored for every site and every patient.

4/30/2023 5 pm Update:

All but one hospital covered by ROH have been connected to the Intelerad network.  A connection to the remaining hospital is imminent.  

With these connections established, ROH can extend remote coverage, particularly overnight,  in order to negate the need for diverted patients.

Outpatient centers are in the process of being connected to the Intelerad network at this time.  Please note that each outpatient center will be contacted directly as connections are established.  

PowerScribe dictation servers are now online and are being configured.  FINAO is trouble shooting any issues as they arise with the new PowerScribe environment.

4/30/2023 8 am Update:

Preliminary reads were conducted yesterday and this morning on the Intelerad system.  Additional facilities were connected to the Intelerad network overnight, and work continues around the clock to connect all remaining facilities.

FINAO and ROH are working with scheduled radiologists and technologists to communicate the current workflow changes.

Configuration of Powerscribe dictation software on the new servers continues.

All, 

Our teams established limited remote access to a new Intelerad connection today, primarily for STATs, ER and Inpatient interpretations.  These preliminary interpretations are being logged as notes within the new Intelerad (new URL; new accounts being built).  Final reports will be completed for each of these patients. These interpretations must be communicated directly with the appropriate clinicians and technologists are working with the designated radiologist to accomplish this workflow. 

Work continues to re-establish the PS 360 server and to establish remaining connections across all hospitals and outpatient centers.  We will communicate directly with each center as these connections are established. Please continue to know our team is working around the clock and we will communicate directly with you as soon as your site is live.

FINAO is connecting individual workstations to the new server environment as facilities are added to the network.  This will enable remote preliminary reads.  We will notify each site individually as connections are established.  PowerScribe dictation software is not yet functional, and preliminary reads must still be communicated to the referring physician at this time.

FINAO established additional connections overnight to specific sites.  These connections were successfully tested and able to receive data (studies).  Configuration adjustments on the Intelerad servers were made.  FINAO continues to work on establishing additional connections to facilities. 

Nuance PowerScribe servers are built and pending completion of software installation.

4/28/2023 5 pm Update:

Installation of Intelerad software on new offsite servers has been successfully implemented.  With this milestone completed, connections to facilities will begin to be reestablished.  FINAO has successfully established its first connection between the new servers and a facility with others actively in progress.

Configuration of the Nuance PowerScribe voice recognition platform is being completed in parallel.

Work will continue around the clock this weekend.  The FINAO environment rebuild remains the highest priority of key partners, including Intelerad, Nuance, Traveller, and two new datacenter partners. 

ROH will maintain its current coverage providing preliminary onsite interpretations over the weekend.

4/28/2023 12 pm Update:

We await the imminent completion of eight (8) new, offsite Intelerad servers.  Intelerad will immediately begin configuration of these servers and loading software.  As soon as humanly possible, we will relay the estimated time to completion.  Completion of this step allows radiologists to view and access facilities’ current images for preliminary interpretation. This is the foundational step toward full restoration of service.

Traveller continues to rebuild VPN’s and various required internet connections across the FINAO environment.  As these come online, designated radiologists will notify facilities.

We will provide the next update before 5PM, and we will have morning, midday and before 5PM updates through the weekend.

Lastly, ROH, FINAO and our entire team genuinely appreciate the overwhelming outpouring of help, offers to assist, and understanding from our referring physicians, our facilities, and their dedicated staff.

4/28/2023 8 am Update:

Work continues around the clock to configure servers both at a new Huntsville data center as well as at an out-of-town center, with extensive Intelerad experience. Once these new servers are live and configured, we will communicate when Intelerad begins loading its software. Thereafter, ROH should have a timeline when a basic reading solution, via Intelerad, will be functional. Similar parallel efforts are occurring with Nuance (PS360) regarding establishing the voice recognition platform.

Work also continues with Traveller to reestablish the needed VPN connections with facility modality servers. ROH has seen progress with Traveller network connections in the past twenty-four (24) hours. The first shipment of hardware arrived at the Traveller data center via expedited transport just after 8:00 PM last night. Unfortunately, vendor analysis of the equipment in proximity to the fire revealed internal temperatures greater than 180 degrees and that equipment is likely not salvageable.

Again, long-term patient data remains safe but, unfortunately, inaccessible at long term data storage centers. Analysis regarding any data in transit from facility modality servers, to Traveller, before transferring to long-term storage will occur once access to Intelerad is reestablished.

ROH will maintain its current coverage providing preliminary interpretations for facilities with Intelerad.

4/27/2023 4 pm Update:

We started to see early signs of some limited progress.  The Team has one server on-line and Traveller is working to reestablish numerous VPN connections.  The system is not yet operational and, unfortunately, we do not have an ETA.

The delivery of the first shipment of new hardware is expected by 8PM tonight.

Unfortunately, sites that utilize FINAO Intelerad for long term storage cannot access any archived images to burn CD’s.  All Physicians are advised to ensure they have physical copies (CD, etc.) of any needed imaging before patients are scheduled for any surgery or procedure.

ROH radiologists continue to provide preliminary interpretations and all imaging studies will be interpreted with a final report once the system is live.

Again, we appreciate your understanding. Please know that our IT team is exhausting every available option to restore service.

4/27/2023 12 pm Update:

Traveller Data Center reports that their DNS servers that provide internet connectivity for the center are coming online.  Thereafter, Traveller  specialists will begin re-establishing VPN connections with facilities. 

FINAO has a total of eight (8) replacement ‘blade servers’ arriving in Huntsville via courier by 8PM tonight.  The FINAO team, Traveller and Intelerad are working around the clock to re-establish PACS services.  Unfortunately, we cannot provide an ETA at this time. 

We plan to provide consistent updates at 8AM, 12 noon and 5PM daily until further notice. Please share this communication with your staff and physicians.

4/27/2023 8 am Update:

We experienced a significant fire with auxiliary servers at the Traveller data center.  Although the impacted servers were not in the FINAO environment, the heat and soot rendered the FINAO / Intelerad servers non operational. 

Foremost, all long-term patient data is safely stored at multiple redundant sites.  Unfortunately, the Traveller data center facility was the gateway to these long-term storage sites. 

Gene, the entire FINAO team, Traveller and Intelerad worked through the night and continue to work on the quickest viable solution for ROH radiologists to resume reads in a PACS environment.  Currently, radiologists can only provide preliminary interpretations at the modality.  

Unfortunately, all interpretations will be delayed and any ‘screening’ imaging (screening mammography, calcium scoring, lung cancer screening exams) cannot be addressed in the immediate future.  These studies will be interpreted as soon as possible. 

ROH will be working with the FINAO team today investigating all viable options including the use of any available legacy PACS.  

Please share this communication with imaging sites, referring physicians and, where needed, with patients, whose interpretations will be delayed. 

ROH apologizes for any and all inconveniences this disaster caused your patients, your referring physicians and your facility.  Please know we will work to a viable solution as quickly as possible.
